Your new Role:

Bring your passion for teamwork, outstanding patient care and work alongside a diverse and supportive team! The Nurse Navigator Generalist role within Mental Health provides community services to patients via clinics located at Logan Central Community Mental Health Centre, Wembley Road and Princess Alexandra Hospital, Woolloongabba. The service is committed to the provision of services that at patient centred and recovery focussed. The purpose of the role is to provide advanced specialist clinical and professional practice, facilitating a safe and efficient patient journey across the continuum of care through applying the Nurse Navigator principles of coordinating patient centred care, creating partnerships, improving patient outcomes, and facilitating systems improvement

About You:

You will understand Metro South Health's values and have a genuine desire to join our team! You will contribute to our growth with your passion and expertise in Mental Health nursing with the ability to:

  • Demonstrate advanced knowledge of contemporary nursing theory, practice, treatments and outcomes.
  • Work both autonomously and within a team environment.
  • Provide educational leadership and expertise, supporting a learning environment by mentoring and promoting team development opportunities to enhance and assure Person Centred Care.
  • Exercise excellent communication skills, strong patient focus and high level of attention to detail.
  • Possess over 5 years of mental health specialist clinical care management and/or evidence of autonomous advanced skills, knowledge, and experience.
