Become a part of an exciting opportunity as a Social Worker – Senior (Hospital in the Home) with Allied Health Services. You will deliver high level clinical Social Work services within the Hospital In The Home service to ensure optimal assessment, clinical intervention, and future care planning occurs for health service consumers as part of a broader multidisciplinary team.

About you:

In your new role you will: 

• Exercise independent clinical judgement and more complex decision making in areas such as psychosocial assessments; short and long term case management; counselling; crisis intervention for a broad range of clinical presentations.
• Provide a leadership role in the clinical assessment and intervention planning for service recipients from a Social Work and multidisciplinary team perspective, ensuring timely identification of client's needs, service provision, and transition/discharge planning within the work unit.
• Lead and manage staff by fostering and committing to patient safety and quality in the delivery of health care by maintaining and evaluating safety and quality practices and inititiatives.
