About us: Cairns and Hinterland Hospital and Health Service covers an area of 142,900 square kilometres. We go as far as Tully in the south, Cow Bay in the north and Croydon in the west.
About your role:
- Conduct comprehensive psychosocial assessments, develop appropriate intervention plans, and achieve outcomes, informed by social work theory and contemporary knowledge of social determinants of health; medical terminology; legislation; resources; services and policy requirements.
- Translate information from clinical assessments and diagnostic conclusions into appropriate social work interventions.
- Provide advocacy to address client needs and ensure equity and access to services.
- Initiate and participate in multidisciplinary case conferences and patient/family/carer meetings.
- Provide clinical support and education to less experienced staff, students and multidisciplinary team members, commensurate with experience.
- Actively participate in and contribute to research, education and professional development programs and activities.
