Get paid to live in paradise and join Allied Health Services team as a Social Worker (Hospital in the Home). Deliver social work clinical services to patients/carers and families. Ensure assessment and interventions including discharge planning occur in line with models of care, service objectives and in consultation with the broader multidisciplinary team.

About your role:

• Conduct comprehensive psychosocial assessments, develop appropriate intervention plans, and achieve outcomes, informed by social work theory and contemporary knowledge of social determinants of health; medical terminology; legislation; resources; services and policy requirements.
• Translate information from clinical assessments and diagnostic conclusions into appropriate social work interventions.
• Provide advocacy to address client needs and ensure equity and access to services.
• Initiate and participate in multidisciplinary case conferences and patient/family/carer meetings.
• Provide clinical support and education to less experienced staff, students and multidisciplinary team members, commensurate with experience.
• Actively participate in and contribute to research, education and professional development programs and activities.
