About us: Cairns and Hinterland Hospital and Health Service covers an area of 142,900 square kilometres. We go as far as Tully in the south, Cow Bay in the north and Croydon in the west.
About you:
In your new role you will:
- Provides specialised clinical expertise, greater experience, leadership and/or specialised skills in the wider health system and with integrating care.
- Assume Nurse Navigator practice for a patient cohort with a high disease burden and complexity in health management.
- The Nurse Navigator focusses on vulnerable patients with multiple co-morbidities, high acute service usage, increased unscheduled care and demonstrates capabilities that support the effective coordination of patients with complex care needs across all care providers in the health journey.
