The Clinical Nurse (CN) will be able to practice autonomously and provide leadership in clinical decision making. They are responsible for professional leadership in the absence of the Nurse Grade 7 and an opportunity to work collaboratively to lead practice changes and clinical innovation.

The Transition Care Program (TCP) is a national program which provides short term support and active management for older persons after an acute or subacute episode with an aim to increase the person's level of function through goal orientated care that has a therapeutic approach.

The TCP forms part of the Community and Preventive Health (CPH) Directorate, a diverse group of service units delivering care and interventions outside the acute hospital setting in most cases. These services comprise all aspects of the care and life continuum including population health, rehabilitation, prevention, early detection, management, reducing disease progression and complications. Services are delivered in a variety of settings such as residential aged care facilities, community-based centres, satellite and outreach services, mobile vans, hospitals and in people's homes.

Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:

  • Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
  • Deliver clinical nursing services often in a home environment in addition to the provision of case management services to ensure a co-ordinated package of care.
  • Fulfil the responsibilities of this role in accordance with Sunshine Coast Health Values.
  • Actively participate in a working environment supporting quality human resource management practices including employment equity, anti-discrimination, work health and safety and ethical behaviour.

Remuneration value up to $105 520 p.a. p/t, comprising salary rates $54.44 - $58.30 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (Permanent Part-Tme position, 60.8 hrs p.f.).
