About the role
Temporary full time opportunity for an experienced hospital Pharmacy Assistant to contribute to the RBWH Pharmacy Patient Flow Team by providing ward-based clinical support to pharmacists undertaking Collaborative Pharmacist Medication Prescribing (CPMP). This is the first in Queensland dedicated team for CPMP with Pharmacy Assistant support essential to ensuring the success of this service. The role delivers advanced Pharmacy Assistant activities and supports the training and development of the Pharmacy Assistant team
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Exercise a high degree of autonomy, initiative and judgment in assisting the provision of a pharmacy service by supporting the clinical pharmacist within a clinical unit and managing the facility requirements accordingly.
- Obtain patient-specific information on pre-admission medication use and clinical information from primary healthcare providers for pharmacist assessment.
- Undertake a Best Possible Medication History (BPMH) for admitting patients and assist in preparing Medication Action Plans (MAP) in readiness for pharmacist review and completion of CPMP.
- Coordinate discharge medicine management including supply, preparation of discharge medication records and liaison with primary healthcare providers under the supervision of a pharmacist.
- Coordinate timely supply of inpatient medicines for newly admitted patients.
