The Specialist Outpatients Unit team are seeking an Outreach Coordinator to join them. The position will be responsible for enabling the delivery of a variety of outreach services across CHHHS, including rural and remote sites, and other HHS’s, including but not limited to TCHHS and THHS by coordinating the engagement, credentialing, data, contracts, service agreement and funding/revenue mechanisms across a range of specialties.

About your role:
- Work in collaboration with the Care Coordination Service Centre to ensure at risk patients are provided with appropriate levels of support to facilitate their engagement with the health service.
- Implement and direct change management strategies and activities by coordinating and providing expert advice and assistance to the CHHHS in relation to outreach activities /services.
- Provide high level advice and support to outreach clinicians which includes providing advice and guidance on critical issues impacting outreach services within CHHHS.
- Manage functions associated with coordinating CHHHS outreach clinics in collaboration with existing outreach administration staff including wait listing of patients, booking and rescheduling for appointments, managing cancellations and using the relevant information system.
