Your new role:
As a Nurse Navigator – Disability, you'll deliver advanced nursing care, coordination and leadership for patients with complex disability needs across Logan Hospital and the surrounding catchment. You'll work collaboratively with internal and external providers to improve patient outcomes, enhance system efficiencies and lead service integration.
You will:
- Coordinate comprehensive care for patients with complex disability-related conditions.
- Provide advanced clinical leadership and consultation to internal and external stakeholders.
- Develop and support multidisciplinary partnerships to ensure continuity of care across health services.
- Empower patients and families through education, self-management support and health literacy.
About you:
You are a compassionate, collaborative and highly skilled nursing professional with advanced clinical and leadership capability in disability care. You'll bring an ability to coordinate complex care across systems while driving innovation and improving patient experiences.
You will demonstrate:
- Extensive experience in disability or complex care management.
- Advanced clinical knowledge, including understanding of NDIS, guardianship, restrictive practices and strengths-based care.
- Proven ability to build partnerships across primary, secondary and tertiary services.
- Strong leadership, communication and change management skills.
