Your new role:
As the Senior Social Worker – Healthy Mob (Identified), you will provide advanced-level social work services within a multidisciplinary primary health care team. Based at the Southern Queensland Centre of Excellence, your focus will be on supporting Aboriginal and Torres Strait Islander clients at risk of chronic disease.
In this role, you will:
- Deliver advanced social work interventions for clients with complex needs.
- Lead culturally safe, holistic care that addresses social, emotional, and health needs.
- Provide high-level clinical advice and support to colleagues and stakeholders.
- Mentor and supervise less experienced social workers.
- Contribute to quality improvement and service development initiatives.
This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.
About you:
You are a passionate and culturally responsive professional with the ability to make a real impact in community health. You bring both advanced social work expertise and leadership qualities, with a commitment to improving health outcomes for Aboriginal and Torres Strait Islander peoples.
To succeed in this role, you will have:
- A degree in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
- Demonstrated experience delivering complex social work interventions in primary health care or community settings.
- Proven ability to engage effectively with Aboriginal and Torres Strait Islander clients and communities.
- Commitment to culturally safe, client-centred care.
- Completed or progressing towards Registration with Medicare Australia to be able to bulk bill for services provided.

About the Southern Queensland Centre of Excellence in Aboriginal and Torres Strait Islander Primary Health Care
The Southern Queensland Centre of Excellence in Aboriginal and Torres Strait Islander Primary Health Care is a Queensland Health service for Aboriginal and Torres Strait Islander people. Located in Inala, 18 km Southwest of Brisbane CBD, our clinic is staffed by GPs, nurses, allied health, and Aboriginal health workers, and visiting medical specialists. We also provide health-related services at a community level, and our research team is dedicated to finding and publishing evidence about how to improve the health of our patients.
