The Community Chronic Conditions Service (CCCS) aims to prevent morbidity associated with chronic and complex conditions, prevent hospital admissions, reduce length of stay and prevent premature admission to Residential Aged Care.
Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:
- Provide leadership and direction for workforce planning and capability strategies for a multi-disciplinary team to achieve alignment, enhancement, and efficiency in the delivery of quality patient care and service. Represent and advocate for Allied Health at a facility/directorate level.
- Provide managerial leadership in collaboration work cooperatively within a multidisciplinary team, particularly in collaboration with Discipline Directors, Allied Health Managers and Team Leaders / Operational Managers across the Sunshine Coast Health.
- Collaborate with senior management to ensure effective cooperation within and across work unit/s to enhance service delivery, quality outcomes and/or patient services.
Remuneration value up to $172 615p.a., comprising salary between $144 991 - $151 289 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP5).
