An exciting opportunity exists for an Allied Health Intake Coordinator (HP4) or Clinical Nurse Intake Coordinator (NRG6) in our Transition Care Program that provides short-term care that seeks to optimise the functioning and independence of older people after a hospital stay.

Transition care is goal-oriented, time-limited, and therapy-focused. It provides older people with a package of services that includes case management, low intensity therapy such as physiotherapy and occupational therapy, as well as social work, nursing support or personal care to maintain and improve physical and/or cognitive functioning.

Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:

  • Work collaboratively across the multiple sites and with a wide range of HHS and NGO stakeholders, to effectively manage and coordinate referrals from the public and private hospital sector across the district and other TCP services, to ensure quality patient outcomes in a time pressured and changeable environment.
  • Develop a suitable care plan reflective of the client's clinical needs, their goals and wishes; and collaboratively plan and coordinate any discharge services, equipment and transport needs with the client and their carers, the inpatient and TCP treating teams.

Remuneration value up to $78,680p.a., comprising salary rates $64.64 - $69.55 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1) or HP4) (part time position, 38hrs p.f.).
