The purpose of this position within the Community Transition Care Program is to apply physiotherapy skills and knowledge in the assessment and treatment of clients in the local geographical population to promote, maintain and rehabilitate functional ability and improve quality of life.

About the role
The Community Transition Care Program operates seven days a week with extended opening hours – 0800 hours to 1700 hours including public holidays. Only nursing services are rostered weekends and public holidays. Further extension of operating hours may occur based on client and hospital and health service need.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver quality physiotherapy clinical services across a variety of caseloads in a Community Transition Care home-visiting model in accordance with prescribed professional and ethical standards. Physiotherapy clinical service delivery is aligned with a restorative focus in a Community Transition Care environment.
- As experience builds, apply increasingly independent clinical judgement to more complex clinical decision making, under the clinical practice supervision or professional guidance of a more senior clinician.
- As experience builds provide case management services to Community Transition Care clients.
- Develop clinical practice knowledge and expertise through active learning within a team and by engaging in professional development activities.
