About us:

The Medical Care Coordinator Clinical Nurse is a role that provides advanced problem-solving skills, expert planning, and coordination skills with the responsibility for enhancing flow for patients admitted under General Medical teams across Rockhampton Hospital including the optimisation and coordination of clinical care to provide patients with a safe, timely, effective, and streamlined discharge experience. The Clinical Nurse works closely with both internal and external partners including but not limited to Shift Coordinators, Senior Medical Officers, Transit Lounge staff, Imaging Department, Queensland Ambulance Service, various Community and Primary Health Services and the Central Queensland Patient Access and Coordination Hub Nurse Managers.

About the role:

The Clinical Nurse (CN) is required to lead and deliver person centred nursing care within a dynamic health environment at an advanced level based on the Nursing and Midwifery Board of Australia – Registered Nurse Standards of Practice. They are required to provide clinical leadership for clinical practice, education and research. This position is responsible for coordination of patient care and discharge planning of General Medical patients admitted to Rockhampton Hospital.

The Clinical Nurse is required to apply critical thinking across all the 5 domains as well as the expectation to work collaboratively to coordinate and support practice changes and clinical innovation. 

About you:

  • Must hold a registration as a nurse with the Australian Health Practitioner Regulation Agency and a current practicing certificate.
  • A relevant postgraduate qualification or working towards is desirable.

What we offer:

Total remuneration value of up to $97,536 p.a. is comprised of:

  • An hourly rate between $54.44 -$58.30 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
