Clinical Nurse (Discharge Planner)

We are seeking an experienced and compassionate Clinical Nurse (Discharge Planner) to join our dedicated multidisciplinary team in Bundaberg. In this pivotal role, you will coordinate safe, timely, and effective discharge planning for patients transitioning from hospital care to home or community services. You will work collaboratively with medical, nursing, and allied health teams to ensure continuity of care and optimise patient outcomes, while supporting families and carers through the discharge process. Bundaberg is surrounded by pristine beaches, national parks, and the iconic Great Barrier Reef Marine Park. The region is famous for its year-round sunshine, the Mon Repos turtle rookery, and its thriving local food scene.

- To provide expert clinical and patient-centred care within the Palliative Acute Rehabilitation, Acute Stroke Unit (PARAS) by supporting the facilitation and provision of quality and cost effective nursing services which are consistent with contemporary standards and evidence-informed practice.

Our offer: You will receive a competitive remuneration package value up to $131,900 p.a., comprising salary between $107,960 - $115,604 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (NRG6(1)) (Permanent Full Time, 76 hrs p.f.)
