Role details

The Division of Medical Services at the Mackay Base Hospital is seeking applications for a Nurse Practitioner Candidate (Renal) to join their team on a temporary basis.

Your opportunity

The Nurse Practitioner Candidate (Renal) will work closely with the Nurse Practitioner Renal to provide a comprehensive (non-dialytic) kidney program which includes interventions to delay progression of kidney disease and minimise complications as well as detailed communication, shared decision making, advance care planning, and psychological and family support for consumers experiencing end staged kidney disease.

This role will lead, support and role model a patient centred approach to evidence-based care and service planning, supporting the health services for our community. This service model seeks to engage patients via multiple modalities including face-to-face, telephone and telehealth to ensure timely access to care.

About the Team

The Mackay HHS Renal Service provides management of adult patients with acute kidney injuries and chronic disease, including dialysis and workup for transplant and post-transplant management. The multidisciplinary team provides outpatient care to over 1300 CKD patients in addition to renal replacement therapy, through advanced holistic patient care in the inpatient, outpatient, and community settings, including comprehensive assessment, planning, implementation and evaluation of patients requiring care. Renal Services patients may be reviewed within the Renal Unit, Specialist Outpatients Department, Renal Home Therapies Unit or via telehealth services to where possible prevent Emergency Department presentations and prevent hospital admission where practical.

The Nurse Practitioner Candidate will work as part of a broader multidisciplinary team to provide patient focused goal driven care.
