Your new role:
As a Senior Social Worker (ABI TRS), you will provide high-level clinical case management and discipline-specific services to patients with acquired brain injury across hospital, home, and community settings. Working within a highly functioning interprofessional team, you'll apply advanced clinical judgement, contribute to service development, and support high-quality, evidence-based rehabilitation care.
You will also play a key role in leadership, education, and service improvement initiatives, helping shape best practice across the ABI TRS continuum of care.
Key Responsibilities
- Deliver senior-level clinical case management including assessment, intervention, evaluation, and discharge planning
- Act as a clinical reference point within the ABI TRS team, responding to complex and non-routine clinical demands
- Apply advanced, independent clinical judgement to complex decision-making with minimal supervision
- Coordinate care across hospital, home, and community environments, including liaison with funding bodies and external agencies
About you:
You are an experienced and motivated Senior Social Worker with a strong background in complex clinical case management and rehabilitation. You thrive in interdisciplinary environments, communicate with confidence, and are passionate about delivering compassionate, person-centred care to people with ABI and their families.
You bring leadership capability, a commitment to continuous improvement, and the ability to balance clinical excellence with service development and mentorship.
Skills & Experience
- Demonstrated high-level experience in clinical case management and ABI or complex rehabilitation settings
- Strong ability to prioritise, manage, and coordinate care in a fast-paced, complex clinical environment
- Experience providing clinical supervision, education, and support to clinicians and students
- Demonstrated involvement in quality improvement or service development initiatives
