About the role
The Pharmacist Senior – DISCO provides Ipswich Hospital with a targeted patient flow focussed pharmacy service across Friday, Saturday, Sunday, and Monday through the coordination of discharge activities to ensure patients are safety discharged.
The position functions with a high degree of independence providing clinical intervention and advice, while coordinating the flow of patient discharges across Ipswich Hospital. The role offers the successful applicant an opportunity to be an integral part of a dynamic team delivering clinical pharmacy services to consumers of West Moreton Health.
About you
Our ideal candidate will be someone who can demonstrate the following:
- High level knowledge, expertise and skills in contemporary pharmacy practice and knowledge of hospital pharmacy service delivery and medication management.
- High level ability to participate in the development, implementation, review and maintenance of internal systems, processes, and work practices.
- Ability to apply professional clinical evidence to support continuous improvement of local service delivery.
- Provide clinical practice supervision to staff, assistants, and support staff, to ensure the maintenance of clinical standards.
- Ability to form effective professional relationships with clients, colleagues, and stakeholders to inform and influence clinical outcomes and/or encourage behaviour change.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
