About the role

We are seeking an experienced Senior Health Worker (Identified) to join the Connecting Care to Country team and support improved health and wellbeing outcomes for Aboriginal and Torres Strait Islander peoples.

This advanced‑level role works autonomously, applying in‑depth knowledge of the health system and Aboriginal and Torres Strait Islander Primary Health Care to deliver culturally safe, holistic care. The role supports individuals, families and communities through proactive health planning, clinical service delivery and long‑term care coordination.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Delivering a range of clinical primary health care services to Aboriginal and Torres Strait Islander clients and communities.
  • Assessing, treating and supporting clients through culturally appropriate health care programs.
  • Anticipating needs and setting long‑term health goals for clients and the community.
  • Maintaining health care systems and providing expert advice and support to multidisciplinary teams.
  • Using specialist skills or extensive experience in Aboriginal and Torres Strait Islander Primary Health Care to strengthen service delivery and outcomes.

This role is classified in accordance with the Aboriginal and Torres Strait Islander Health Workforce (Queensland Health) Certified Agreement (No. 1) 2019 (EB1) and forms part of an evolving workforce structure. While future reviews may result in changes to terminology or classification, there will be no loss of employment conditions or remuneration for employees transitioning to revised structures.
