Your opportunity:
- This is a case management and clinical service delivery position incorporating assessment, care planning, coordination, and treatment of clients during their episode of care whilst on the Transition Care Program.
- The Health Practitioner provides direction and support to other staff while promoting professional standards of practice and conduct and, is responsible to contribute towards the achievement of unit goals and standards. The Health Practitioner may be allocated portfolio responsibilities to facilitate this.

Our offer:
You will receive a competitive remuneration package value up to $66,283 p.a., comprising salary rates $39.87 - $58.60 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Part Time, 38 hours p.f.).
