Role details
The Aboriginal and Torres Strait Islander Health Unit at the Mackay Base Hospital is seeking applications for a Clinical Nurse Consultant (Connected Communities Pathways) to join their team on a temporary basis.

Your opportunity
The Clinical Nurse Consultant will work collaboratively with the Nurse Practitioner and other team members in a nurse led multidisciplinary team for First Nations and vulnerable populations with chronic conditions utilising a prevention model of care with a focus on Type 2 Diabetes Mellitus in persons aged 15 years and older. This model algins with health equity strategies and the organisation's strategic and operational plan to improve Aboriginal and Torres Strait Islander health. The model is implemented within the primary health care and prevention setting, with an aim to reduce avoidable ED presentations, reduce potentially preventable hospitalisations and provide more timely and appropriate access to care, closer to home where clinically appropriate within the Mackay Hospital and Health Service District.

The Aboriginal and Torres Strait Islander Health Unit is an essential element of the MHHS support services and ensures the development and delivery of culturally appropriate services and models of health care to Aboriginal and Torres Strait Islander people and families.
