What your day could look like
As an Advanced Health Worker, you'll be central to how we support First Nations people across the region. From coordinated care and advocacy to education and partnership, you'll work with a multidisciplinary team to deliver care that is culturally safe, accessible, and responsive.
Every day, you'll contribute by:
- Coordinating individual and family care plans that improve access, continuity and outcomes.
- Advocating through referrals, home visits and case conferencing that remove barriers to care.
- Leading culturally safe health education that builds knowledge, confidence and self‑determination.
- Strengthening partnerships and data quality, guiding teams in identification and respectful practice.

Here's what will help you thrive in this role:
- Aboriginal and/or Torres Strait Islander person for an Identified position.
- Diploma or Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (or Practice).
- Strong communication, advocacy and report writing; confidence working across services.
- Current ‘C' class driver licence and readiness to meet role-specific health and safety requirements.
You'll stand out from the crowd if you have experience in rural or remote settings and/or additional training in primary health care practice or health promotion.
