About us:

Gladstone Hospital is a 108-bed hospital and offers a range of general and specialist inpatient and outpatient services including Emergency, General Medicine and Surgery, Basic Orthopaedic, Cancer Care, Clinical Investigation, Obstetrics, Gynaecology, High Dependency and Coronary Care, Palliative Care, Medical Imaging, Pathology, Paediatrics and a Renal Dialysis Unit.

Gladstone Hospital has approximately 400 full time equivalent staff delivering services across acute, community and primary care. There is a 42-bed mixed General Surgical and Medical Ward and a four bed High Dependency Unit where patients may be held pending retrieval, or for close observation not possible on the wards. A new Transit Lounge has recently opened to improve patient flow throughout the hospital.

The hospital has a busy outpatient clinic reviewing a wide variety of surgical and medical conditions. The pharmacy team provides patient centred dispensing and clinical pharmacy services to all areas of the hospital. Your day will be filled with meaningful interactions, providing the highest standards of pharmaceutical care to directly enhance patient outcomes.

About the role:

Participate in a comprehensive clinical pharmacy service to the Gladstone Hospital. The candidate will have the opportunity to participate in daily clinical pharmacy rounds within the clinical care environment throughout Gladstone Hospital.

  • Participate in the provision of clinical pharmacy services to all treatment units including (transit lounge) of the Gladstone Hospital. Including undertaking patient medication history interviews, medication reconciliation, formulation of pharmaceutical care plans and patient counselling.
  • Supervise and participate in efficient and effective medicines dispensing within agreed procedures (including prescription validation and evaluation to ensure medicine therapy is safe, appropriate and effective based on patient's available history, approval/authority for use of non-List of Approved medicines, accurate labelling, final product verification and patient counselling).
  • Communicate with patient, carers and community healthcare providers to ascertain accurate and complete medication histories on admission as well as to communicate changes in therapy on discharge including written and verbal information to patients and community healthcare providers.

About you:

  • Appointment to this position requires possession of a Degree in Pharmacy and eligibility for registration as a pharmacist with the Australian Health Practitioner Regulation Agency. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.

What we offer:

Total remuneration value of up to $132,567 p.a. is comprised of:

  • An hourly rate between $43.68 – $58.59 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
