About the role

This role is based within both Ripley Specialised Inpatient Services facility and Ipswich Hospital. The successful candidate will be expected to work between both sites. The CHIP nurse is an expert in the holistic assessment of older patients; is able to work collaboratively with the multidisciplinary team and is able to develop positive relationships with internal and non-government service providers. The CHIP service also incorporates the Sub and Non-Acute Patient (SNAP) Coordinator role on a rotational basis.

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Demonstrated advanced clinical knowledge in the area of discharge planning delivering expert nursing care in complex situations while providing support and direction to other team members.
  • Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
  • Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
