Join our Transition Care Program as an Occupational Therapist supporting older people to recover safely and confidently at home following hospital discharge. Working within a supportive multidisciplinary team, you’ll deliver person‑centred therapy and case management that promotes independence and wellbeing. This role offers variety and impact, with opportunities to support clients across Mareeba, Cairns and the Cassowary Coast.

About us: 

The Transition Care Team delivers high‑quality, low‑intensity allied health, nursing and case management services to older people following discharge from acute and sub‑acute hospital settings. Clients are community‑based and have been assessed as eligible for residential care, with Transition Care supporting their recovery, independence and decision‑making.

About your role: 
 
  • Provide consistent, safe and efficient occupational therapy and case management services, within a multidisciplinary team, with infrequent need for direct clinical practice supervision.
  • Demonstrate resilience and problem-solving abilities while working in varied and demanding caseload
  • Provide ongoing development and maintenance of internal and external networks and proactively developing self, including maintaining contemporary knowledge of the profession, management and organisational strategies.
  • The position reports operationally to the Team Leader for Transition Care and professionally to the Director of Occupational Therapy.
