About us:
The Community Specialist Palliative Care Service provides high quality patient focused care and timely response to the specialist palliative care and end of life health care needs for palliative patients across Central Queensland Hospital and Health Service. The team comprises of a Staff Specialist Palliative Care, Clinical Nurse, Social Worker, and Administration Officer staff that deliver high-quality care that is culturally safe and person-centred care responsive to enable the success of achieving better outcomes for our consumers and better value care ‘needs based' approach to improves access to specialist palliative care service within the community.
About the role:
The Aboriginal and Torres Strait Islander Health Practitioner will provide a range of clinical, primary health care services to Aboriginal and Torres Strait Islander clients and communities including delivering specific health care programs, assessing and treating clients, maintaining health care systems, and providing advice and support whilst under clinical supervision in order to contribute to better health outcomes for Aboriginal and Torres Strait Islander people.
- Support Aboriginal and Torres Strait Islander people to navigate the health system, ensuring they have continuity of care across the healthcare continuum.
- Engage with community members to strengthen community involvement in health service delivery.
- Provide practice and cultural support to other Health Professionals to build cultural competence and responsiveness and assist them to navigate Aboriginal and Torres Strait Islander communities and customs.
About you:
- Appointment, and future increment progression, between paypoints HWF3.2 to HWF3.5 requires current registration with the Aboriginal and Torres Strait Islander Health Practice Board, including a Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice.
- Appointment, and future increment progression, between paypoints HWF3.3 to HWF3.7, or HWF3.8 requires current registration with the Aboriginal and Torres Strait Islander Health Practice Board, including a Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice and Diploma of Aboriginal and/or Torres Strait Islander Primary Health Care Practice or Diploma of Aboriginal and/or Torres Strait Islander Primary Health Care.
- Under Section 25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander community.

What we offer:
Total remuneration value of up to $121,350 p.a. is comprised of:
- An hourly rate between $39.00 - $53.64 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
