What your day could look like
As the Social Worker Senior, you'll be a cornerstone of care for patients and families across hospital, outpatient and community settings in and around Chinchilla. From complex assessment to collaborative discharge planning, you'll help people access the right support, closer to home.
Every day, you'll contribute by:
- Delivering advanced assessments, interventions and case coordination that enable safer care, timely transitions and better outcomes.
- Partnering with patients, carers and clinicians—using clear handover and advocacy—to set goals, solve problems and reduce barriers.
- Leading quality and service improvements, using data and evidence to refine models of care and uplift performance across sites.
- Coaching and supervising early‑career social workers and students, building capability and a culture of learning.

Here's what will help you thrive in this role:
- Recognised Social Work degree and eligibility for AASW membership.
- Proven experience managing a complex caseload across acute, outpatient and community settings.
- Exceptional communication and teamwork skills, with the confidence to lead change and share expertise.
- Current ‘C' class driver licence and willingness to travel between facilities.
You'll stand out if you have rural generalist experience and/or postgraduate study in a relevant field.
