Your new role:

This role plays a vital part in improving health outcomes for Aboriginal and Torres Strait Islander communities through culturally appropriate chronic disease management, education, and care coordination.

  • Deliver evidence-based, culturally appropriate clinical services, health promotion, and chronic disease management activities for individuals and families
  • Coordinate care and liaise with hospital, community health, and social service providers to support integrated service delivery
  • Engage with local communities and stakeholders to identify priorities and develop targeted health strategies and action plans
  • Support clients to navigate services and take ownership of their health management, while contributing to data collection and reporting

About you:

We are seeking a dedicated and culturally capable professional who is passionate about improving health outcomes and supporting Aboriginal and Torres Strait Islander communities.

  • Strong cultural understanding and sensitivity, with knowledge of community protocols and the social determinants impacting health outcomes
  • Demonstrated communication and interpersonal skills, with the ability to build trust and work effectively with clients, families, and multidisciplinary teams
  • Proven ability to plan, problem-solve, and manage workload independently while contributing to team-based care
  • Clinical knowledge and experience in chronic disease management (e.g. diabetes, respiratory, cardiac), with a commitment to continuous learning and improvement
