Your new role:
As a Psychologist/Case Manager, you will deliver high-quality, evidence-based mental health services within the PAH South Adult Community Mental Health Team. Working in a collaborative, multidisciplinary environment, you'll support consumers through assessment, intervention, and recovery-focused care while contributing to service improvement and clinical excellence.
In this role, you will:
- Deliver comprehensive psychological assessments and interventions
- Manage a diverse caseload with a focus on recovery-oriented care
- Collaborate within a multidisciplinary team to optimise outcomes
- Engage with consumers, families, and external stakeholders
- Contribute to discharge planning and continuity of care
- Maintain accurate clinical documentation and service reporting
- Support students and junior staff through supervision and education
- Participate in quality improvement and research activities

About us:
The function of the Community Care Team South is to provide specialist services to people aged 18-65 with severe and enduring mental health and addiction presentations whose needs cannot be met by primary health services and/or the private sector or non-government organisations (NGOs) alone. The Community Care Team works in partnership with the person, their family/carers and stakeholders to plan, assess, deliver and evaluate services using the AIMS Framework (Assessment, Intervention, Monitoring, Step-up/down). The teams has a multi-disciplinary approach where clinicians work collaboratively with the person on their recovery goals, based on individual needs to enable the person to feel confident in managing their health and wellbeing independent of secondary services and to support transitioning from tertiary mental health services to ongoing care in the primary/private and NGO sectors.
