Your new role:
As a Clinical Nurse Consultant within the Dementia Outreach Service, you will provide advanced clinical leadership and specialist expertise to deliver high-quality, person-centred dementia care across community and residential aged care settings.
- Lead and deliver advanced clinical nursing practice in cognitive impairment and dementia, including assessment, care planning and consultancy
- Provide expert advice, education and support to multidisciplinary teams, patients, carers and aged care staff
- Drive quality improvement, evidence-based practice and clinical governance initiatives across the service
- Work autonomously and collaboratively to enhance service delivery, reduce hospital admissions and improve patient outcomes
About you:
You are an experienced and highly skilled nursing professional with a passion for dementia care and a strong commitment to clinical excellence and patient-centred outcomes.
- Registered Nurse with current NMBA registration and demonstrated advanced clinical experience in dementia or cognitive impairment care
- Proven ability to lead, influence and collaborate effectively within multidisciplinary teams
- Strong critical thinking, decision-making and problem-solving skills, with a focus on evidence-based practice
- Excellent communication, education and stakeholder engagement skills, with a commitment to continuous learning and quality improvement
