Your new role:

This is your opportunity to take the lead in coordinating complex care for children and young people experiencing acute mental health challenges. As a Nurse Navigator, you'll drive seamless care, reduce system barriers, and improve outcomes across the continuum.

In this role, you will:

  • Lead end-to-end care coordination across hospital and community services
  • Deliver brief, targeted interventions to young people and families in crisis
  • Streamline pathways and reduce fragmentation across the care journey
  • Partner with multidisciplinary teams and external stakeholders
  • Champion service innovation and system improvement initiatives

About you:

You are a confident, driven mental health nurse who thrives in complex environments and is passionate about improving care for young people.

You will bring:

  • Advanced experience in child and youth mental health settings
  • Proven ability to lead, influence and coordinate complex care
  • Strong communication skills with families, teams and stakeholders
  • Systems thinking with a focus on improving patient journeys
  • A commitment to innovation, evidence-based care and better outcomes

Metro South Addiction and Mental Health Services provide inpatient, hospital-based and community mental health and community alcohol and drug services for all age groups across a number of campuses.
