About the role
The role of Team Leader for the Preventative Integrated Care Service (PICS) provides operational management and allied health clinical leadership to the PICS service.
PICS addresses a gap for a target population of high-risk chronic condition patients requiring specialist intervention and coordinated care in their most vulnerable periods to improve quality of life, avoid unplanned admissions, prevent possibly preventable hospitalisations (PPH) and reduce length
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated high level knowledge of contemporary preventative and hospital avoidance service delivery with the ability to integrate evidence-based knowledge into practice.
- Demonstrated ability to lead a multidisciplinary team utilising the principle of contemporary human, financial and material resource management.
- Demonstrated high level oral and written communication and interpersonal skills to facilitate equitable and effective engagement with staff and internal and external stakeholders.
- Demonstrated ability to provide strategic leadership for service planning and service improvement for the service.
- Demonstrated ability to provide clinical leadership for the development of professional standards for Prevention and Hospital Avoidance practice in West Moreton Health.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
