About the role
- Deliver advanced level clinical Social Work services to clients of Post-Acute Care Services (PACS) and other Community and Oral Health (COH) Home Hospital Services.
- Provide advanced level clinical leadership to Social Workers within COH Home Hospital Services, inclusive of Hospital in the Home (HITH), Post Acute Care Service (PACS), Geriatric Evaluation and Management in the Home (GEMITH), Virtual Ward and Community Palliative Care across multiple sites within Metro North Health (MNH).

- Provide advanced, specialised Social Work services of a highly complex and varied nature independently in multiple community settings to patients with complex psychosocial needs and varied health conditions, working across speciality areas within the Home Hospital Service.
- Apply interdisciplinary and transdisciplinary knowledge to provide clinical advice and direction to Social Workers and the multidisciplinary team to improve client experience and outcomes.
- Contribute to professional governance of COH Social Workers by providing clinical leadership and direction in the development, implementation and review of contemporary clinical practice standards, models of service delivery and clinical pathways to ensure best outcomes for clients.
