About us:
The Sub-Acute Chronic Care Rehabilitation team provides interdisciplinary rehabilitative care to clients with a range of complex chronic diseases. These services also work to address unnecessary medical admissions and support early discharge by enhancing the quality and availability of community based proactive collaborative care. This service uses the interdisciplinary client centred model as described in the Sub-Acute Chronic Care and Rehabilitation service profile, with student placement and clinical education embedded in this core business.
About the role:
As the Clinical Nurse (Chronic Disease), you will provide expert assessment, care planning, education, coordination and management for clients with chronic conditions and those at risk, while promoting healthier lifestyle outcomes.
Not only that, you will also deliver clinical services with a strong focus on leadership, education, management and quality improvement. Working predominantly in a clinical capacity, you'll support senior nursing staff while guiding Registered Nurses, Enrolled Nurses and other team members in complex care environments.
Practicing autonomously, leading clinical decision-making, and coordinating care will allow you to step into this professional leadership role!
About you:
What do we need from you? Simple! Provide your registration as a Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a current annual practising certificate.
A few other things to help boost your application:
- High level knowledge of legislation, standards, policies and procedures relevant to contemporary nursing practice and specific to area of practice.
- Demonstrated ability to identify and resolve clinical issues using well developed problem-solving skills.
- Ability to source and develop specialised information and health education for clients, community groups and other health care providers.
- Proven ability to work effectively as a member of a team with an ability to facilitate, promote and adapt to change.
What we offer:
Total monthly remuneration value of up to $11,604 p.m. is comprised of:
- An hourly rate between $57.48 - $61.55 p.h.
- 12.75% employer superannuation contribution
