About us:
Cairns and Hinterland Hospital and Health Service (CHHHS) provide a level five Cardiology Service. The Cardiac Outreach and Outpatient service is part of the multidisciplinary team comprising Nurse Practitioner, Clinical Nurse Consultants, Clinical Nurses and Health Workers delivering sub-acute and post-acute care.

About your role:
- Deliver advanced cardiac care in heart failure titration clinic in accordance with accepted professional practices, legal and ethical requirements with a high degree of independence.
- Provide specialist/advanced clinical advice to facilitate service delivery within heart failure service to ensure effective service delivery and quality outcomes.
- High level skills and knowledge in problem solving, establishing priorities and working autonomously within a multidisciplinary environment.
- Demonstrated ability to deliver culturally safe care, communicate effectively and use highly developed interpersonal skills to facilitate good teamwork in a multi-disciplinary and cross-cultural working environment
- Implement strategic and operational planning responsibilities in heart failure service.
