About the Team

The Assistant Director of Nursing - Health Equity, Access and Care Coordination (HEACC) will be responsible for providing contemporary and evidence-based models to facilitate equitable and seamless access and care coordination to patients and families across Children's Health Queensland. 

The Health Equity, Access and Care Coordination Unit brings together services across the continuum of care that facilitate access and/or support all patients and families of CHQ to effectively navigate their healthcare journey. Services within the Health Equity, Access and Care Coordination unit include:

  • CHQ Connected Care Program
  • Navigate Your Health
  • Indigenous Health Liaison Officers.

The unit is further focused on facilitating and coordinating equitable access and care for Aboriginal and Torres Strait Islander patients and families.

About the Role

The Assistant Director of Nursing - Health Equity, Access and Care Coordination provides strategic, professional and operational leadership across the HEACC portfolio within the Division of Medicine to advance health equity, culturally safe care and integrated care coordination.  The role has a strong focus on clinical and cultural leadership, ensuring that nursing staff and Identified Health Liaison Officers are supported to work collaboratively and effectively to meet the needs of our patients and families. 

The role is accountable for strengthening culturally safe professional practice, clinical governance and workforce capability across divisional services, embedding cultural safety principles, professional standards and equity frameworks into everyday practice.
