Your new role:
As a Clinical Nurse Consultant within Chronic Disease services, you will provide advanced clinical leadership and expert cardiac nursing care across a community-based, multidisciplinary environment.
- Deliver high-level clinical expertise in cardiac nursing, providing both direct and indirect patient care across the care continuum
- Lead and support evidence-based practice, quality improvement initiatives, and clinical governance within the service
- Collaborate with multidisciplinary teams to enhance patient outcomes, reduce hospital admissions, and support community-based care
- Provide education, mentorship, and clinical guidance to staff while contributing to service innovation, research, and workforce development
About you:
You are an experienced and motivated nursing professional with advanced clinical expertise in cardiac care and a passion for delivering person-centred outcomes.
- Registered Nurse with current registration and demonstrated advanced clinical knowledge in cardiac nursing practice
- Proven ability to lead, influence, and collaborate effectively within multidisciplinary healthcare teams
- Strong critical thinking, problem-solving, and decision-making skills with a commitment to evidence-based practice
- Experience in education, mentoring, and driving quality improvement initiatives in complex clinical environments
