An exciting opportunity exists to join the Older Persons Integrated Health Service as a Nurse Practitioner. This role provides advanced nursing care using a case management model and coordinates comprehensive patient care. The Nurse Practitioner delivers advanced assessment and management, provides emergent responses to Residential Aged Care Facilities to prevent ED presentations, and supports community‑based care, including care in patients’ homes, as part of an interdisciplinary team.Job details

About us:
The Older Persons Integrated Health Service (OPIHS) integrates care of the older person across the emergency department, in-patient acute and sub-acute care, and community settings. OPIHS is a seven-day service which incorporates the clinical streams of Geriatric Emergency Department Intervention (GEDI), Older Persons Liaison Service (OPELS), RACF support services (RaSS) and Older Persons Enablement And Rehabilitation for Complex Health Conditions (OPEN ARCH). The aim of the service is to provide timely and appropriate care for the older person which will avoid unnecessary community deterioration in health, reduce avoidable hospital presentations, reduce unnecessary hospital admissions, and facilitate early and safe discharge from hospital.

About your role:
- Hold responsibility and accountability for clinical decisions and actions consistent with the agreed scope of practice for the role.
- Provide an advanced and extended patient-centred consultancy practice participating in direct and indirect patient care provision across the care continuum within the context of the multidisciplinary team.
- Proactively conduct clinical service that is enhanced and expanded by autonomous and accountable practice while applying relevant legislation, guidelines and standards.
- Provide clinical leadership, mentorship and support for nurse practitioner candidates and other relevant staff.
