Your new role:
This is a senior leadership role responsible for driving excellence in clinical care, service delivery and operational management within the Community Hospital Interface Program (CHIP).
- Lead and coordinate CHIP services to deliver high-quality, person-centred care and optimal patient outcomes
- Provide strategic and operational leadership across workforce, financial and resource management
- Drive service improvement, innovation and evidence-based practice aligned with organisational priorities
- Foster a high-performing multidisciplinary team culture through leadership, mentoring and workforce development
About you:
You are an experienced nursing leader who combines advanced clinical expertise with strong operational capability and a passion for improving patient outcomes.
- Demonstrated advanced clinical knowledge and experience in complex care, including discharge planning and hospital avoidance models
- Proven leadership capability with experience managing teams, resources and service delivery in a healthcare setting
- Strong communication, negotiation and stakeholder engagement skills across multidisciplinary environments
- Experience in quality improvement, data-driven decision making and implementing evidence-based practice
