About the role
Permanent part time opportunity to provide high-level clinical knowledge and skills to deliver specialist contemporary, evidence-based social work clinical services and leadership as a member of the multidisciplinary team to adult consumers with complex mental health issues, their families/carers and relevant stakeholders in a mental health Hospital in The Home Service.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Apply high-level knowledge, skills and independent judgement in the assessment and treatment planning for adult consumers with complex needs who are inpatients of The Prince Charles Hospital (TPCH), Hospital in The Home and their families/carers, in accordance with the Australian Association of Social Workers Professional Practice Standards and Code of Ethics.
- Apply high-level advocacy within the healthcare system including facilitating linkages with key agencies to address client needs and ensure equity in access to services.
- Work with the leadership team regarding consumer patient flow whilst ensuring appropriate treatment and discharge planning is identified an implemented.
