About the role
Deliver high-quality, evidence-based clinical services including assessment, intervention and evaluation to people experiencing homelessness with complex mental health within a five-kilometre radius of the Brisbane CBD. Work within a multidisciplinary team using an assertive outreach approach, collaborating with stakeholders, families and carers to provide trauma-informed, recovery-oriented care. Contribute to improving health outcomes and reducing homelessness through coordinated, cross-sector support.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver competent-level social work / psychology / occupational therapy clinical services to consumers experiencing homelessness with complex mental health needs based on valid and reliable evidence, in accordance with professional practice, ethical, and mental health standards to ensure culturally appropriate, consumer-focused outcomes. X
- Actively participate in the multidisciplinary team by providing social work / psychology / occupational therapy specific clinical advice and education, and by contributing to the development of care plans and discharge planning to achieve optimal outcomes for consumers.
- Monitor and report on clinical practices and outcomes within the clinical service area and actively participate in quality and service improvement / research activities that enhance the delivery of clinical services and support achievement of key performance indicators.
