The Cairns Integrated Palliative Care Service delivers culturally appropriate care across hospital and community settings, including telehealth and Residential Aged Care Facility (RACF) outreach, supporting people with life-limiting illness, their families, and carers through coordinated, multidisciplinary care and strong community partnerships.
About your opportunity:
In this position you will…..
- As part of a multi-disciplinary team and utilising the palliative approach provide a culturally responsive service in collaboration with other healthcare professionals, develop case management and healthcare plans for Aboriginal people, and support clients in complying with their care plan.
- Holistic assessment and screening of physical health and social emotional wellbeing of the palliative care patient, their carers and their family.
- Support clients in self-management, including the use of non-pharmacological treatment strategies used in palliative care.
