About the role
Provide high-level social work services across multiple clinical programs as an annual leave relief role, delivering psychosocial support to patients and families. Work within a multidisciplinary team to achieve positive health outcomes and flexible service delivery.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver high-quality social work services, including assessment, intervention, and advocacy to support patient outcomes
- Work collaboratively within multidisciplinary teams to provide patient-centred care and optimise service delivery
- Apply evidence-based practice, professional judgement, and problem-solving to manage complex caseloads
- Contribute to clinical education, supervision, and support of staff and students
- Participate in quality improvement, research, and service development initiatives
