About the Role
As a Social Worker in our Acute Care Team, you will:
- Provide person-centred social work services to individuals experiencing acute mental health challenges.
- Collaborate with a multidisciplinary team to deliver evidence-based support and interventions.
- Lead timely and accurate documentation to meet legislative and care standards.
- Work within the values of Metro North Health to ensure safe, culturally responsive, and outcomes-driven care for patients and their families.
- Participate in quality improvement and service development initiatives to improve care delivery.
Our Acute Care Team is a 24-hour service, requiring participation in a roster providing extended service coverage. The role may involve working across different Metro North Health facilities, ensuring indivualised, comprehensive care is delivered to those who need it most.
About You
You're a compassionate and resourceful social worker with:
- Demonstrated experience in managing complex mental health cases and interventions.
- Knowledge of key clinical systems and an ability to adapt quickly.
- Strong collaboration and communication skills to work effectively within multidisciplinary teams.
- A commitment to continuous improvement and culturally safe, responsive healthcare.
