Your new role:
This is an exciting opportunity for an experienced and passionate Clinical Nurse Consultant to provide advanced clinical leadership and specialist expertise in heart failure and cardiology nursing within a community-based multidisciplinary team focused on improving patient outcomes and reducing hospital admissions.
Deliver advanced clinical care, consultancy and leadership in heart failure and cardiology nursing across community and home-based care settings.
Collaborate with multidisciplinary teams to support safe, evidence-based, person-centred care and improve service delivery and patient outcomes.
Lead and contribute to quality improvement initiatives, clinical redesign, education programs and evidence-based practice activities.
Provide expert mentoring, coaching and clinical support to nursing staff while promoting a positive and collaborative team culture.
About you:
We are seeking a highly skilled and motivated nursing professional with advanced clinical expertise in heart failure and cardiology nursing, strong leadership capabilities and a commitment to delivering exceptional patient-centred care.
Registered Nurse with current Nursing and Midwifery Board of Australia (NMBA) registration and demonstrated advanced clinical experience in heart failure and cardiology nursing.
Proven ability to lead clinical practice, provide expert assessment and care planning, and support evidence-based healthcare outcomes across the care continuum.
Strong communication, negotiation and relationship-building skills with the ability to work collaboratively across multidisciplinary teams and community-based services.
Passion for education, quality improvement and innovation, with experience mentoring staff, leading change initiatives and contributing to research and service development.
