About the role
Deliver high-quality care for patients with chronic kidney disease within a specialised outpatient service, working under the supervision of consultant nephrologists. Support the development of a contemporary, patient-centred model of care while contributing to service improvement, clinical governance, and multidisciplinary collaboration.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide clinical care to patients with chronic kidney disease, including assessment, management, and follow-up within scope of practice
- Collaborate closely with nephrologists, nurses, pharmacists, and general practitioners to ensure coordinated care
- Develop and implement comprehensive management plans and communicate effectively with referring clinicians
- Recognise and refer patients appropriately when care exceeds scope of practice or requires escalation
- Contribute to quality improvement, clinical audit, and service development activities
