About us:
The Continuing Care Team provides specialist multidisciplinary assessment, treatment and care for patients who experience a wide range of severe and persistent psychosis, mood, anxiety, behaviour and eating disorders that significantly impact daily functioning and quality of life. Our services are provided across the community to ensure continuity of care.
Interventions for the treatment of mental health presentations in adults can include:
- Biological or pharmacological (e.g. medications and monitoring management regarding prescribed medications for treatment, including Clozapine).
- Psychological (e.g. psychological therapies such as Insight Orientation, Cognitive Behavioural Therapy, Relapse Prevention, psychoeducation, and education to consumer's families).
- Social or vocational (e.g. social skills training).
- Primary Care (shared Care Programs).

About your role:
In your new role you will....
- Provide support, guidance, mentoring, preceptorship, role modelling and promote cooperation and collaborative teamwork.
- Lead a team in conjunction with a Nurse Grade 7 to assist and support with team shift coordination, performance management/review, change management, rostering/staffing and workplace culture.
- Participate in developing and undertaking quality initiatives, clinical audits, clinical trials, and research.
- Provide clinical leadership for professional and clinical practice, education, and research.
