Role details

The Heart Health Team at the Mackay Base Hospital is seeking applications for a Clinical Nurse to join their team on a Permanent basis.

Your opportunity

To foster and enhance the provision of an excellent, patient focused Heart Health service within the Mackay district. The Clinical nurse provides advanced clinical nursing care in complex cardiac disease management through the application of principles of best practice, education, leadership and portfolio management.

Provide cardiac education and support to both inpatient and outpatient clientele at an advanced level, inclusive of Cardiac Care Unit and Cardiac Catheter Laboratory patients and the ability to provide support and education to the multidisciplinary team.

To work within a supportive multi-disciplinary team whilst being responsible and accountable for the co-ordination and delivery of quality patient focused, evidence-based nursing care providing creative leadership, education, direction and support to other team members, nursing staff and students.

About the Team

The Heart Health Service sits within the Cardiac and Cardiac Services Department at Mackay Base Hospital and is committed to the optimisation of health outcomes for people with a range of cardiac conditions ensuring quality of life for the client, their carers and families by the delivery of quality, coordinated and integrated multidisciplinary care across services, settings and time. The Heart Health Team works in close collaboration with the Coronary Care Unit, Cardiac Catheter Laboratory and the Clinical Measurements Unit.

This service provides Education and support to patients and their families, Cardiac rehabilitation, secondary prevention and heart failure management programs across the continuum from hospital to home. Nursing and allied health staff consult with clients in the acute care settings of Mackay Base Hospital as well as provide ongoing follow up care in the community by telephone, Cardiac Exercise program and outpatient/Telehealth clinics.
