About the role

This is an exciting opportunity to deliver advanced, person-centred care within a specialised aged care environment. Working across Cooinda and Gannet House, you will provide expert clinical leadership to support residents with complex, chronic, and multifaceted health needs. In this role, you will utilise your advanced clinical capability to assess, diagnose, and manage resident care, applying a holistic approach that promotes dignity, independence, and quality of life. You will operate with a high level of autonomy while partnering closely with an interdisciplinary team to ensure coordinated and responsive care.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Fulfil the responsibilities of this role in accordance with Schedule 2, Nurses and Midwives (Queensland Health Award) (2015) – Generic Level Statements (GLS) Nurse Grade 8 and the Queensland Health and Metro North Health's commitments and values, in achievement of the organisational goals and Domains of Practice.
  • Demonstrate autonomy, accountability, and competence for expanded practice in providing client-focused care within a nursing model including advanced health assessment of consumer's/patient's clinical condition, development, and implementation of appropriately documented management plan.
  • Provide specialist nursing interventions that demonstrate an expanded level of nursing knowledge and skills in the practice area of specialised aged care.
  • Provide advanced clinical support and leadership through evidence-based practice and professional development, service planning and delivery, and quality initiatives.
  • Evaluate service outcomes with specific indicators and benchmarking and apply appropriate change management strategies as required.
