Your new role:

As the Allied Health Team Leader, you will provide operational management and strategic leadership to an interprofessional Chronic Disease team. You will lead service delivery, workforce planning and quality improvement initiatives while fostering a collaborative and patient-focused culture.

Key Responsibilities

  • Lead multidisciplinary Chronic Disease services and operational team management.
  • Support innovative models of care, service improvement and evidence-based practice.
  • Manage staffing, workforce planning, budgeting and resource allocation.
  • Provide leadership, mentoring and strategic advice to support high-quality patient outcomes.

 

About you:

You are a collaborative and experienced allied health professional with strong leadership capability and a passion for delivering innovative, patient-centred care.

  • Proven leadership experience within multidisciplinary healthcare teams.
  • Strong communication, stakeholder engagement and change management skills.
  • Sound clinical knowledge with a commitment to evidence-based, patient-centred care.
  • Ability to lead service improvement, workforce planning and operational management.

Community and Oral Health Services deliver a broad range of quality community-based care and support services to help people improve and maintain their wellbeing and independence as well as oral health services to adults and children.

We deliver a myriad of services across Metro South including Hospital in the Home, Rehabilitation, Chronic Disease, Acute Care, Dementia Outreach, Oral Health, and specialist residential care along with delivering numerous other vital community health services.
