About the role
Provide advanced clinical nursing care within the Rapid Access to Community Care (RACC) service, delivering timely assessment, care planning, and intervention to support patients at risk of hospitalisation. Work within an interdisciplinary framework to promote patient-centred care and improve community-based health outcomes.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver advanced clinical nursing care including comprehensive assessment, planning, implementation, and evaluation of patient care
- Apply critical thinking and clinical judgement to manage complex and changing patient conditions within the community setting
- Work collaboratively with multidisciplinary teams to provide coordinated, patient-centred care
- Provide education and support to patients, families, and staff to enhance independence and care outcomes
- Contribute to quality improvement, research, and continuous service development initiatives
